MondoIVF Biological Material Transport Protocol
Phase 1: Collection at Origin Clinic
| Patient Consent Verification: Confirm that the clinic has all required patient authorizations for transport. | |
| Identity Match: Verify that straw/vial identification codes match the provided Summary of Records. | |
| Dry Shipper Status: Confirm container is in Vapor Phase and maintain proper upright orientation. | |
| Temperature Logger: Confirm that the data logger is active and recording. | |
| Security Seal: Apply tamper-evident seal and record the specific seal number on the Chain of Custody (CoC). | |
| Documentation: Collect original Chain of Custody and necessary medical/customs reports. |
Phase 2: Airport and Air Transit
| NO X-RAY COMPLIANCE: Ensure a Manual Hand Inspection is performed at all security checkpoints. | |
| Continuous Supervision: Keep the cryogenic container under direct physical custody at all times. | |
| Upright Orientation: Maintain the dry shipper in a vertical position during ground and air transit. | |
| BBA Checklist Fulfillment: Cross-reference all operational steps with the mission-specific BBA Agreement. |
Phase 3: Delivery at Destination Clinic
| Seal Inspection: Present the intact numbered seal to the receiving laboratory personnel. | |
| Biological Verification: Confirm material is frozen and intact upon opening the container at the destination lab. | |
| Data Review: Provide the data logger to the lab for temperature profile verification. | |
| Formal Acceptance: Obtain final signature, time of delivery, and clinic stamp on the CoC form. |
